Release and Waiver Form

We, at My G.I.R.L.S., Inc., recognize that there is a risk of injury when participating in the sports of gymnastics, dance, and cheerleading. While we do our very best to ensure that these things do not happen, there is always a risk. We always take safety precautions as well as teach (and remind) the children of what those safety rules are. Parents are asked to remind their children of the risk of injury, and therefore, the importance of following the rules in class at all times.

I understand that cheerleading has an inherent danger in participation, and that in spite of all precautions, and accident preventatives, injuries do occur, I intend to be legally bound and do, hereby for myself, my heirs, executors, and administrators, waive, release and forever discharge all claims which may arise now or in the future which I may accrue against My G.I.R.L.S., Inc., and its respective administrators, directors, agents, officers, volunteers, employees, other participants, any sponsors, advertisers, and if applicable, owners and lessors of premises on which the activity takes place for any and all injuries suffered by me while attending and participating in cheerleading tryouts, practice, activities, and competitions. In order that I receive the necessary medical treatment in the event of an injury or illness, I hereby authorize My G.I.R.L.S., Inc., to obtain medical treatment for myself for such injury or illness during any event, and I hold My G.I.R.L.S., Inc. harmless in their exercise of the authority. To the best of knowledge, I do not have any diseases or injuries that would medically prohibit my participation in practices, tryouts, competition, and participating in cheerleading activities for My G.I.R.L.S., Inc. I further certify that I have read and understood the above statements and that the information provided is truthful to the best of my knowledge.WAIVER/RELEASE FOR COMMUNICABLE DISEASES INCLUDING COVID-19ASSUMPTION OF RISK / WAIVER OF LIABILITY / INDEMNIFICATION AGREEMENTIn consideration of being allowed to participate on behalf of My G.I.R.L.S., Inc. athletic program and related events and activities, the undersigned acknowledges, appreciates, and agrees that:Participation includes possible exposure to and illness from infectious diseases including but not limited to MRSA, influenza, and COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist; and, I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and,I willingly agree to comply with the stated and customary terms and conditions for participation as regards protection against infectious diseases. If, however, I observe and any unusual or significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and,I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS (insert name of sports organization) their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“RELEASEES”), WITH RESPECT TO ANY AND ALL ILLNESS, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF RELEASEES OR OTHERWISE, to the fullest extent permitted by law.With the above in mind, I fully understand the policies set in place at My G.I.R.L.S., Inc., and release all rights and claims to the greatest extent allowed by the law for damages that my child may have against My G.I.R.L.S., Inc.

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT. I consent to allow my child to participate in My G.I.R.L.S., Inc activities in accordance to the class, clinic, and/or team registered for.

FOR PARTICIPANTS OF MINORITY AGE (UNDER AGE 18 AT THE TIME OF REGISTRATION)

This is to certify that I, as parent/guardian, with legal responsibility for this participant, have read and explained the provisions in this waiver/release to my child/ward including the risks of presence and participation and his/her personal responsibilities for adhering to the rules and regulations for protection against communicable diseases. Furthermore, my child/ward understands and accepts these risks and responsibilities. I for myself, my spouse, and child/ward do consent and agree to his/her release provided above for all the Releasees and myself, my spouse, and child/ward do release and agree to indemnify and hold harmless the Releasees for any and all liabilities incident to my minor child’s/ward’s presence or participation in these activities as provided above, EVEN IF ARISING FROM THEIR NEGLIGENCE, to the fullest extent provided by law.
